New PIP

Click the DocuSign button below if you are a Service Provider starting a New PIP for a client. All forms, including the worksite approval, are included. You will receive a copy, and we will reach out once the worksite is approved. Wait time is typically one business day.

If you are a Service Provider or Family under SDP, fill out our affiliate agreement & worksite approval below. Please do not fill out if you have already completed one in the past. If you are using a previously approved worksite, you do not have to fill out this information.

Onboarding Steps

  • Step 1.

    Send a Pre-approval email with the client’s information to the Regional Center.

  • Step 2.

    Once the regional center confirms, schedule an IDT meeting for the client.

  • Step 3.

    Fill out the onboarding paperwork & affiliate agreement. You will receive a copy immediately. We will send out a worksite approval email no later than one business day.

  • Step 4.

    Contact your Regional Center with your filled-out PIP Face Sheet to send to the Regional Center. They will connect both of us, and we will be able to take it from there.
